Lost or Stolen Canadian Passport

If your valid Canadian passport has been lost or stolen, it must be reported immediately and replaced by following the steps below.

1. Report Your Passport Lost or Stolen

You must report your passport as lost or stolen to the Passport Program right away.
Once reported:

  • The passport is cancelled permanently
  • It cannot be used again, even if it is later found

2. Complete Form PPTC 203

You must complete the Statutory Declaration Concerning a Lost, Stolen, Inaccessible, Damaged or Found Canadian Travel Document (PPTC 203).

This declaration is mandatory for all replacement passport applications and requires you to explain when, where, and how the passport went missing.

3. Submit a New Passport Application (NOT a Renewal)

Lost or stolen passports are not eligible for the simplified renewal process — even if you still have a photocopy of your previous passport.

To replace your passport, you must submit a General Adult Passport Application.  This application requires a qualified.

4. Guarantor Requirements

A guarantor must:

  • Have known the applicant for at least two years
  • Live in Canada or the United States
  • Be able to communicate in English or French

Guarantor Duties

The guarantor must confirm the applicant’s identity by:

  • Complete and sign the guarantor section (Section 2)
  • Sign the back of one passport photo
  • Provide a copy of their own ID (front and back)

Accepted Types of Guarantors

  • Canadian Guarantor: Holds a valid Canadian passport
  • Professional Guarantor: A member of a designated profession recognized by Passport Canada
  • No Guarantor Available: You may complete a Statutory Declaration in Lieu of Guarantor, signed by an authorized official
